EU funding for integrated care for people with chronic kidney disease (CKD)

SLOVENIA, November 11 - As part of this EU-financed project, the National Institute of Public Health of the Republic of Slovenia and the University Medical Centre Ljubljana will work together to launch a prevention programme for people diagnosed with CKD at primary level and design integrated clinical pathways for managing persons with CKD. Thus, all structures and health services at various levels of the health care system and beyond will be integrated, which will pave the way towards better care for persons with CKD and their self-care. Moreover, the project will help improve access to care and equal treatment of persons with CKD in Slovenia.

The project Integrated care for people with chronic kidney disease (CKD) for improving health literacy and reducing health inequalities – Let's stop CKD will be implemented under Slovenia’s EU Cohesion Policy Programme 2021-2027, under priority Long-term care and health, and social inclusion, and pursues the specific objective Enhancing the equal and timely access to quality, sustainable and affordable services, including services that promote the access to housing and person-centred care including healthcare; modernising social protection systems, including promoting access to social protection, with a particular focus on children and disadvantaged groups; improving accessibility including for persons with disabilities, effectiveness and resilience of healthcare systems and long-term care services.

The project is worth a total of EUR 2,523,913.24; most of the funding for the project, i.e., EUR 1,564,504.10, will come from the European Social Fund Plus.
